By entering and/or utilising the gym facilities you acknowledge that there are risks and dangers inherent in physical exercise in any environment and you must declare that you know of no reason, medical or otherwise, why you / your minor should not exercise. You agree that by participating in physical exercise or training activities, you / your minor do so entirely at your own risk. Any recommendation for changes in diet including the use of food supplements, weight reduction and/or body building enhancement products are entirely your responsibility and you should consult a physician prior to undergoing any dietary or food supplement changes.

You declare that a physician has approved for you / your minor to engage with training such as that conducted within our facility. You agree that you are voluntarily participating in these activities and use of these facilities and premises and assume all risks of injury, illness, or death. Maze BJJ accepts no responsibility for injuries, however caused, neither loss nor damage of personal property which you / your minor may take with you onto the premises. You / your minor also agree to abide by the standards of acceptable conduct whilst in the facility and not to enter prohibited areas. Maze BJJ assumes no liability for persons who undertake physical activity, and if in doubt after completing this questionnaire consult your doctor / your minors doctor prior to physical activity. In consideration of being allowed to participate in a fitness assessment and personal fitness programme provided by Maze BJJ.

The rule is that users should train with at least one other person in case of injury or illness. Any person who circumvents the procedures designed to prevent this rule being broken do so at their own risk. Users are recommended to have some form of medical insurance to cover injury. Users are advised that it is the responsibility of each individual to ensure that he or she has achieved such levels of fitness and general health as will allow the use of the gym without injury or accident. Maze BJJ can accept no liability for injury or accident other than in accordance with our insurance cover for liability insurance. You / your minor are only allowed to use equipment in accordance with manufacturer guidance and once you / your minor have completed the enrolment form and induction classes.

For Martial Arts Members

I understand that by participating in training BJJ (unarmed combat), which involves engaging in dangerous activity which may subject to injury. I am in full knowledge of the risks, nonetheless, agrees to enter into this agreement and herby waives any claim that I / my minor may have against Maze bJJ or its affiliates.

Use of Images

I hereby give consent to Maze BJJ to take and use images (photographs or videotape) or sounds recordings of me / my minor and to disclose information about me to or in any public media, including radio, television, Internet or print, or in any publication. I understand that the intended use of such images and information is for advertising, marketing, fundraising or promotional purposes of Maze BJJ. I hereby waive the right to or interest in the images or the confidentiality of the information disclosed to the public, as contemplated in this release. I acknowledge that this consent to use images and authorization for release of information is being made solely for the benefit of Maze BJJ and without any expectation of compensation or other benefit to me / my minor. To the extent that any benefit accrues or might accrue to Maze BJJ from the use of images or disclosure of information, I hereby and forever waive any interest in or claim to such benefits. I hereby release and forever discharge Maze BJJ (including without limitation all corporate affiliates and officers, directors, trustees, employees, staff members and agents) from any and all claims, liability, actions, suits, demands, costs, expenses or indebtedness arising out of, related to, or in any way connected with the use of images or disclosure of the information and materials described herein, and I hereby waive all rights and interest in and to such information and materials. I have been informed that this authorization is voluntary and is subject to revocation at any time, except to the extent that action has been taken in reliance thereon, by notifying Maze BJJ in writing to: Unit 4, Diskin centre, Golden Island,
Ankers Bower, Athlone.

Terms and Conditions

Membership fees will be charged on a month-to-month basis using the billing information provided upon joining, until you provide Maze BJJ with 30 days written notice of intent to cancel. If you (the buyer) choose to end the month to month charging then decide to re-join at a later date, you will start a new monthly agreement. If you decide to cancel your account, it is required that you (the buyer) send your cancellation notice by mail/email in order to prove that you did send notice. Membership fees are non-refundable and non-transferable. To cancel your monthly membership payment, you MUST follow the above procedure. If purchasing personal training sessions or a group class pass, I understand that the session’s and/classes are also non-refundable and non-transferable.

Privacy & Security Policy

Commitment to Data Security

Your personally identifiable information is kept secure. Only authorised employees, agents and contractors (who have agreed to keep information secure and confidential) have access to this information. All emails and newsletters from our sites allow you to opt out of further mailings. Maze BJJ will never sell trade, rent, exchange or otherwise share your personal information with any other person, company or organisation.

I attest that:

- I / my minor am not experiencing any symptom of illness such as cough, shortness of breath or difficulty breathing, fever, chills, repeated shaking with chills, muscle pain, headache, sore throat, or new loss of taste or smell.

- I / my minor have not travelled internationally within the last 14 days while there is a travel ban within Ireland.

- I / my minor have not travelled to a highly impacted area within the last 14 days.

- I hereby release and agree to hold Maze BJJ harmless from, and waive on behalf of myself / my minor, my heirs, and any personal representatives any and all causes of action, claims, demands, damages, costs, expenses and compensation for damage or loss to myself and/or property that may be caused by any act, or failure to act of the salon, or that may otherwise arise in any way in connection with any services received from Maze BJJ. I understand that this release discharges Maze BJJ from any liability or claim that I / my minor, my heirs, or any personal representatives may have against the gym with respect to any bodily injury, illness, death, medical treatment, or property damage that may arise from, or in connection to, any services received from Maze BJJ. This liability waiver and release extends to the gym together with all owners, partners, and employees.

So far as permitted by law, I do hereby waive, release and discharge Maze BJJ and their agents, employees, representatives, from any and all claims or liabilities for injuries or damages to my person / my minors person and/or property or omission of any of those mentioned or others acting on their behalf, arising out of or connected with my / my minors participation in any activities, programmes or services of Maze BJJ or the use of any equipment provided and/or recommended by Maze BJJ. I have been informed of, understand and am aware that any exercise programme, whether or not requiring the use of exercise equipment, is a potentially hazardous activity and that I / my minor is voluntarily participating in these activities and using equipment and machinery. 

You acknowledge that you have carefully read this “waiver and release” and fully understand that it is a release of liability. You expressly agree to release and discharge the trainer or instructor from any and all claims or causes of action and you agree to voluntarily give up or waive any right that you / your minor may otherwise have to bring a legal action against the trainer or instructor for personal injury or property damage. To the extent that statute or case law does not prohibit releases for negligence, this release is also for negligence. If any portion of this release from liability shall be deemed by a Court of competent jurisdiction to be invalid, then the remainder of this release from liability shall remain in full force and effect and the offending provision or provisions severed here from. By signing this release, I acknowledge that I understand its content and that this release cannot be modified orally.

By submitting this application, I affirm that the facts set forth in it are true and complete. I understand that if I / my minor am accepted as a member of the gym as accept the disclaimer and rules noted above, any false statements, omissions, or other misrepresentations made by me on this application may result in cancellation of my membership.
