Maze BJJ Waiver, Membership Terms and Consent Form

By entering or using Maze BJJ facilities, you confirm that you understand and accept the risks involved in physical exercise, gym training, and martial arts.

If this form is for a minor, references to “I”, “me”, or “my” also apply to the minor and their parent or guardian.

Health and Fitness

I confirm that I, or my minor, have no medical or other reason that would prevent safe participation in exercise, gym training, Brazilian Jiu-Jitsu, or any other activity at Maze BJJ.

I understand that physical exercise and martial arts involve risk of injury, illness, or in rare cases, death. I take part voluntarily and at my own risk.

I confirm that I am responsible for ensuring that I, or my minor, have a suitable level of fitness and general health before using the gym or taking part in classes.

If I have any doubts about my, or my minor’s, ability to participate safely, I agree to consult a doctor before taking part.

Any advice about diet, supplements, weight loss, or body-building products is my responsibility to assess, and I should consult a doctor before making any major dietary or supplement changes.

Use of Facilities and Equipment

I agree that I, or my minor, will only use gym equipment safely, according to manufacturer guidance, and only after completing the required enrolment form and induction classes.

I agree to follow Maze BJJ rules, behave appropriately, and stay out of prohibited areas.

Members should train with at least one other person where possible, in case of injury or illness. Anyone who avoids or bypasses this safety rule does so at their own risk.

Maze BJJ recommends that members have suitable medical insurance to cover injury.

Maze BJJ is not responsible for loss or damage to personal belongings brought onto the premises.

Martial Arts Training

I understand that Brazilian Jiu-Jitsu and martial arts training involve physical contact and unarmed combat.

I understand that these activities may be dangerous and may result in injury.

I choose to participate, or allow my minor to participate, with full knowledge of these risks.

To the fullest extent permitted by law, I waive any claim that I, or my minor, may have against Maze BJJ, its owners, instructors, staff, employees, agents, or affiliates arising from participation in martial arts training.

Liability Waiver

I understand that all exercise, training, classes, programmes, and use of equipment at Maze BJJ carry risk.

To the fullest extent permitted by law, I release Maze BJJ, its owners, instructors, employees, agents, representatives, and affiliates from any claims, liabilities, costs, damages, or expenses arising from injury, illness, death, medical treatment, or property damage connected with:

• use of the gym or equipment;
• participation in classes, programmes, or services;
• fitness assessments or personal training;
• martial arts training;
• advice or recommendations provided by Maze BJJ.

This release includes claims relating to negligence where permitted by law.

If any part of this waiver is found to be invalid by a court, the rest of the waiver will remain in effect.

I understand that this waiver is a release of liability and that it cannot be changed verbally.

Health and Illness Declaration

I confirm that I, or my minor:

• am not currently experiencing symptoms of illness such as cough, shortness of breath, fever, chills, muscle pain, headache, sore throat, or loss of taste or smell;
• have not travelled internationally within the last 14 days where a relevant travel ban or restriction applies in Ireland;
• have not travelled to a highly impacted area within the last 14 days.

Use of Images and Recordings

I give Maze BJJ permission to take and use photographs, videos, and sound recordings of me, or my minor.

I understand these may be used for advertising, marketing, fundraising, promotional materials, social media, websites, print, television, radio, or other public media.

I understand that I will not receive payment or other compensation for the use of these images or recordings.

I waive any rights or claims relating to the use of these images, recordings, or related information.

I release Maze BJJ, including its owners, officers, employees, staff, agents, and affiliates, from any claims connected with the use of these images, recordings, or information.

I understand that this consent is voluntary and may be withdrawn at any time by writing to:

I understand that withdrawal will not affect any use that has already taken place before Maze BJJ receives my written notice.

Membership Terms

Membership fees are charged monthly using the billing details provided when joining.

Monthly membership continues until I give Maze BJJ 30 days’ written notice of cancellation.

Cancellation notice must be sent by mail or email so that I can prove notice was given.

Membership fees are non-refundable and non-transferable.

If I cancel and later rejoin, I will enter into a new monthly membership agreement.

Personal training sessions and group class passes are also non-refundable and non-transferable.

Privacy and Data Security

Maze BJJ will keep my personal information secure.

Only authorised employees, agents, and contractors who have agreed to keep information confidential may access this information.

Maze BJJ will not sell, trade, rent, exchange, or otherwise share my personal information with another person, company, or organisation.

Emails and newsletters from Maze BJJ will include an option to unsubscribe.

Final Declaration

By submitting this application, I confirm that all information I have provided is true and complete.

I understand that false statements, omissions, or misrepresentations may result in cancellation of membership.

I confirm that I have read and understood this waiver, membership terms, and consent form.

I understand that by signing or submitting this form, I accept the risks and agree to the terms above.
